Dietary Accommodations and Food Allergies

REQUEST FOR DIET MODIFICATION:

The Texas Department of Agriculture requires that the school nutrition program must provide reasonable accommodations for the reimbursable meals at no additional charge, snacks and a la carte sales when (1) a student's disability restricts his/her diet and (2) the disability is supported by a medical statement signed by a medical authority who is licensed by the State of Texas to write medical prescriptions.  Therefore, the Texas Department of Agriculture requires that the district maintain an updated diet order on file providing these special dietary accommodations.

A completed Physician's Statement if required to accommodate diet modifications.  The Physician Statement is available at the campus nurse's office or the link on this page.  A medical authority who is Licensed by the State of Texas to write medical prescriptions must complete the form.  Submit the completed Physician Statement to your child's campus nurse.  Please ensure that it is completely filled out with all the required information, including appropriate substitutions, so Nutrition Services can promptly process the request and begin dietary accommodations as soon as possible.  Incomplete forms or forms missing information will take longer to process.  Until the form has been processed, students will receive a regular lunch tray or they can bring a lunch from home.

The Physician Statement needs to be renewed only when dietary needs change.  Discontinuation of an accommodation for diet modification can be submitted in writing by medical authority who is Licensed by the State of Texas to write medical prescriptions or child's parent/guardian.  Phone number must be included on the statement from parent/guardian.  Send statement to the school's nurse.  The district Registered Dietitian will call the parent or guardian to verify the request prior to making any change.  Changes to dietary treatment must be in writing by State licensed healthcare professional.

A LA CARTE SALES:

Middle and high school students will be provided their physician's ordered special diet when going through the tray line; however, if a la carte items are selected for purchase the student is responsible for choosing correct foods.

All students in grades (K-6) on special diets will be provided their physician's ordered special diet when going through the tray line.  A la carte items for purchase will be limited (based on student's food allergies).

DISCLOSURE:

Manufacturers provide food labels to the Alief ISD Nutrition Services office.  Product reformulation may occur at any time and may not be known by our department.  In addition, distributors may deliver (on short notice) alternate/substitute products which contain unexpected allergen.  Alief ISD Nutrition Services will make every effort to identify possible allergens in the food we serve including providing tools such as published menus and allergen charts.
